A successful Transition Plan involves not only parents and students, but all appropriate professionals working towards the same outcome.
Program participants and their families begin to plan for their future from the time they enroll with GCRC.
Habilitation training including Socialization, Self-Regulation of Behaviors, Cooperative Play, and Pre-Vocational Skills begin as early as age 6 when children are enrolled in GCRC's after-school and summer programs. Future planning continues through High School until program participants achieve adulthood.
GCRC offers Transition Services through contracts with the Division of Developmental Disabilities and Rehabilitation Services Administration (Vocational Rehabilitation).
GCRC's Transition and Future Planning services complement IEP goals, Behavioral Health objectives, DDD outcomes, and Vocational Rehabilitation plans.
GCRC utilizes best practices for successful transition planning:
- Include a student's preferences, interests, and personal needs.
- Encourage self-awareness to help students understand their individual needs.
- Support students in self-advocacy to effectively participate in their planning meetings.
- Develop goal-setting skills to help students set education and employment goals.
- Encourage students to monitor their own progress and stay accountable.
- Promote self-determination: the ability to decide one's future and path in life.
