Independent Living
Independent Living Supports at the Graham County Rehabilitation Center are designed to empower individuals with developmental disabilities to live fulfilling and self-reliant lives in their own apartments. Our dedicated team of Direct Support Professionals (DSPs) provides personalized assistance and guidance, equipping program participants with the necessary skills and supports to thrive in their independent living journey.
What is Independent Living Supports?
Independent Living Supports focus on providing individuals with developmental disabilities the necessary supports to live independently in their own apartments. At Graham County Rehabilitation Center, we understand that independence means different things to different individuals. We offer customized supports that address the unique needs and goals of each program participant, ensuring they have the tools and resources to live a self-determined life.
Personalized Supports and Skill-Building
Our DSPs work closely with program participants to develop personalized supports and skill-building plans tailored to their specific needs. We offer assistance in areas such as finding suitable housing, budgeting, meal planning and preparation, housekeeping, personal hygiene, accessing community resources, and maintaining a safe and healthy living environment. Through one-on-one guidance and practical instruction, we empower individuals to acquire the skills necessary for successful independent living.
Housing Options
We understand the importance of safe and suitable housing in promoting independent living. Our team assists program participants in exploring housing options that best align with their preferences and needs. We provide guidance on navigating the housing market, accessing affordable housing resources, understanding lease agreements, and connecting with property managers or housing agencies.
Community Engagement
Living independently goes beyond the physical aspects of having an apartment. We also emphasize community engagement and social inclusion as vital components of independent living. Our DSPs help individuals connect with community organizations, recreational activities, social groups, and other opportunities for meaningful engagement. We support program participants in building social networks, fostering friendships, and accessing community resources to enhance their overall well-being.
Health and Safety
Health and safety are essential aspects of independent living. Our team assists program participants in understanding healthcare systems, connecting with healthcare providers, managing medications, and accessing appropriate healthcare services. We also provide education and support in emergency preparedness, personal safety, and general well-being, ensuring individuals have the knowledge and resources to maintain a healthy and secure living environment.
Continued Support and Monitoring
Our commitment to independent living extends beyond the initial transition. We provide ongoing support and monitoring to ensure the well-being and success of program participants in their independent living journey. Our team remains accessible for guidance, assistance, and periodic check-ins to address any challenges, provide additional supports, and celebrate milestones achieved along the way.
